TITLE OF BILL:
An act to amend the education law, in relation to non-patient specific
orders a registered professional nurse may perform
 
PURPOSE OR GENERAL IDEA OF BILL:
To enable registered nurses. (RNs) to carry out certain procedures with-
out a patient-specific order from the attending practitioner.
 
SUMMARY OF SPECIFIC PROVISIONS:
Section one of the bill amends subdivision 4 of section 6909 of the
education law by adding four new paragraphs (i), (j), (k) and (1). (i)
adds electrocardiogram tests to detect signs and symptoms of acute coro-
nary syndrome, (j) adds administering point-ofcare blood glucose tests
to evaluate acute mental status changes in persons with suspected hypo-
glycemia, (k) adds administering test and intravenous lines to persons
that meet severe sepsis and septic shock criteria and (1) adds adminis-
tering tests to determine pregnancy.
Section two of the bill amends subdivision 6 of section 6527 of the
education law by adding four new paragraphs (i), (j), (k) and (1). (i)
adds electrocardiogram tests to detect signs and symptoms of acute coro-
nary syndrome, (j) adds administering point-of-care blood glucose tests
to evaluate acute mental status changes in persons with suspected hypo-
glycemia, (k) adds administering test and intravenous lines to persons
that meet severe sepsis and septic shock criteria and (1) adds adminis-
tering tests to determine pregnancy.
Section three is the effective date.
 
JUSTIFICATION:
Non-patient specific standing orders allow registered nurses to assess a
patient's medical condition and provide timely care, within the RN'
scope, in certain situations without waiting for the attending practi-
tioner's direct order. Standing orders improve patient care, decrease
wait times, and enhance workflow for nurses juggling diverse patient
needs. Standing orders are implemented in a variety of healthcare
settings, including hospital inpatient settings where care teams are
often managing time-critical medical emergencies.
Current law allows RNs to follow non-patient specific standing orders in
several situations, such as administering specified immunization agents
and anaphylaxis treatment agents. This legislation would expand the use
of standing orders by permitting RNs to administer additional procedures
and tests in the hospital setting pursuant to an attending practition-
er's standing order. The expansion would include electrocardiograms
(EKGs) when patients present with symptoms of a possible heart attack;
point-of-care glucose screenings for patients with suspected hypoglyce-
mia; sepsis lab panels and intravenous access for patients that meet
severe sepsis and septic shock criteria; and pregnancy tests prior to
undergoing select imaging studies, operative procedures, and/or adminis-
tration of anesthetic agents. The bill maintains the authority of the
attending practitioner, the hospital, and Health Department regulations,
and protects the role and scope of practice of RNs.
