We need to make sure everyone has access to the health care they need. The new Prescription $aver Card, issued by the New York State Department of Health, is one way to help ensure our residents can afford the medicine they depend on to stay well.
The Health Department has applications for the free card. The program offers participants significant discounts on thousands of prescription medications, providing savings of up to 60 percent on generic drugs and 30 percent on brand name drugs.
This pamphlet provides you with guidelines about eligibility and contact information for the Prescription $aver Card program.

Who is eligible?*
New Yorkers who are disabled under Social Security Administration guidelines or are between
the ages of 50 and 64:
- Annual income less than $35,000 for single individuals
- Annual income less than $50,000 for married couples

* Medicaid recipients and those enrolled in the Elderly Pharmaceutical Insurance Coverage (EPIC)
